ABSTRACT: Antimicrobial

resistance (AMR) has become one of the most significant threats to global public health, with India bearing one of the world’s highest burdens of resistant bacterial infections. Rural India presents unique challenges due to widespread over-the-counter access to antibiotics, limited diagnostic facilities, inadequate antimicrobial stewardship, poor sanitation, and extensive antibiotic use in agriculture and livestock production. These factors collectively accelerate the emergence and spread of resistant microorganisms, compromising the effectiveness of life-saving antimicrobial therapies. The consequences include prolonged illness, increased healthcare expenditure, higher mortality, and reduced productivity among rural populations. This review summarizes the current status of antibiotic resistance in rural India, examines its epidemiology, major drivers, commonly encountered resistant pathogens, socioeconomic determinants, surveillance challenges, and the role of the One Health approach. The review also discusses evidence-based interventions, including antimicrobial stewardship, public awareness campaigns, rapid diagnostic technologies, improved sanitation, strengthened surveillance systems, and policy implementation. Addressing antibiotic resistance in rural India requires coordinated efforts involving healthcare professionals, policymakers, veterinarians, agricultural sectors, researchers, and local communities to preserve antibiotic effectiveness for future generations.
